Mass General Hospital needed a set of online psychiatric evaluation forms and scales for their psych department. The data entered into these forms had to be automatically compiled into a report and formatted in very specific ways to create an official look.

The forms we made use ASP and Javascript to compute scores and compile data which is passed off to Adobe Reader with the FDF Toolkit. The report can then be printed from Reader.

FlamingVan worked directly with doctors to create the forms.
